Family Health Teams
The role of the Community Health Team is to increase access to health and social services and to help people overcome barriers to wellness. Programs designed to overcome these barriers focus on issues such as nutrition, managing chronic diseases, smoking cessation, aging, and mental health.
We are a Family Health Team!
The Family Health Team is a new patient-centred approach to primary health care that brings together different health care providers to co-ordinate the highest possible quality of care for you – the patient.
Designed to give doctors support from other complementary professionals, the Kawartha North Family Health Team will include nurse practitioners, nurses, social worker, and other health care professionals who will work collaboratively, each using their experience and skills so that you receive the very best care, when you need it. A collaborating physician is part of this team.
The Family Health Team approach will allow us to:
- provide better access to care, closer to home
- work as a team to keep patients healthy
- help patients navigate their way through the health care system
- provide primary health care, chronic disease management and self-help tools to improve health
- use 'state of the art' information technology giving providers access to patient information and test results
